LAUNDERING OF SOILED LINEN -1992

 

SERVICE and persons eligible to Tender.- The Deputy Director of
Provincial Health Services Galle, will receive separate sealed tenders
from Sri Lankans or Sri Lankan Firms for the Laundering of Soiled
Linen for the period 01 st January, 1992 to 31st December 1992 (both
days inclusive) to the institution mentioned in Column 2 of the
Schedule hereto.
 

 

2. Tender Deposits.-

(i) A cash deposit of the sum specified in
column 2 of the Schedule hereto should be made at this office in my
favour and receipt obtained.
(ii) Tender Forms.- Tender forms will be issued up to 12 noon
prior to the date of closing of tenders at this office. No tender will be
considered unless it is on the appropriate form.
Application for tender forms by post should be made sufficiently
early to enable prospective tenderers to obtain the forms and submit
tenders before the closing date and time.

 

3. Particulars of Worth.- All tenderers should before applying for
tender forms furnish well in advance of the closing of tenders,
particulars of their worth to me.

 

Failure to Furnish the Particulars of Worth or Proof of Financial
Status May Result in the Forfeiture of the Tender Deposit.

 

4. Particulars of number of pieces to be washed.- Particulars
regarding the approximate number of pieces required to be washed
per mensem are given to Column 3 of the Schedule hereto.

 

5. How Tenders should be forwarded,- All tenders should be
forwarded in duplicate and be enclosed in a cover addressed to the
Deputy Director of Provincial Health Services, Galle. The cover
enclosing the sealed tenders should bear at the left top corner the
nature of the service and at the left bottom corner name and address
of the tenderer. The cover enclosing the sealed tender may be sent by
registered post or deposited in the tender box at this office or handed
over personally to me or to the Accountant who will issue an
acknowledgement of the tender.

 

6. Closing of Tenders.- Tenders will close at this office at 10.00
a. m. on the date mentioned in Column 6 of the Schedule hereto and
will be opened immediately thereafter.

 

7. Opening of Tenders.- Tenderers may be present-When the
tenders are opened . The names of the tenderers as well as the rates
will be read out to the tenderers present. Any tenderer who wishes to
scrutinize a tender will by arrangement with me or my authorized
representative will be permitted to scrutinize the duplicate of any
tender, that has been submitted.

 

8. Validity of Tendered Rates.- The rates quoted by the tenderers
should be valid for the period of contract mentioned in para. 1 above and in no circumstances will the rates be permitted to be varied
during such period unless specifically provided for in the Agreement.

 

9. Tender Deposits.- The selected tenderer will be required to
sign the Agreement within ten days of the acceptance of the tender
after furnishing the required security mentioned in Column 3 of the
Schedule hereto.

 

10. Tender Conditions.- Prospective tenderers should study the
tender conditions on the reverse of the tender forms and comply with
the requirements therein.

 

11. (A) Other particulars-

(i) Facilities of Electricity and Water.-
monthly recovered at the rate assessed by the Regional Secretary
(Assistant Government Agent) of the area will be made from
the contractor’s monthly voucher for facilities if in the nature of
electricity and water.
(ii) Disinfecting Soiled Linen and Transport Charges thereof- In
case where it is required that the soiled linen should be disinfected
before it is washed, the transport charges will be paid for the transport
of soiled linen from an institution to the disinfecting station and back.
(iii) Tenders for institutions in Local Authority areas.- Tenders
for institutions in Local Authority areas should be from those who
maintain laundries in accordance with the regulation, if any, framed
by the Local Authorities.

SUPPLY OF COOKED PROVISIONS
WITHOUT MILK-1992

 

SERVICE and Persons Eligible to Tender.-The Deputy Director of
Provincial Health Services, Galle will receive separate sealed ten-
ders from Sri Lankans or Sri Lankan firms for the supply of cooked
provisions without milk to the Institutions mentioned in Column 1 of
the Schedule hereto for the period 0lst January, 1992 to 31st
December, 1992 both days inclusive.

 

2.
(i) Tender Deposit.-A Cash deposit of the sum specified in
Columns 2 and 3 of the Schedule hereto should be made at the office
of the Deputy Director of Provincial Health Services, Galle and a
receipt obtained. Approved Rural Development Societies or Regis-
tered Co-operative Societies including Multi-purpose Co-operative
Societies and Unions of Multi-purpose Co-operative Societies may
apply for tender forms without making tender deposits. They should
however, furnish the registration number of the Society when mak-
ing their applications for tender forms.
(ii) Tender Forms.-Tender forms will be issued at this office
upto 12 noon prior to the date of closing of tenders during week days
between 9.00 a.m. to 3.00 p.m. No tender will be considered unless
it is on the appropriate form.
Application for tender forms by post should be made sufficiently
early to enable prospective tenderers to obtain the forms and submit
their tenders before the closing date and time. Applications for tender
forms should be made attaching the receipts obtained for the tender
deposits made. Incomplete lenders can be rejected at the discretion
of the tender board.

 

3. Particulars of Worth.-All tenderers before applying for
tender forms must submit to me a worth Certificate issued on a
date within two months immediately before the closing date on
form General 170 obtained from the Regional Secretary (Assistant
Government Agent) of their residential area to the effect that his
worth is not less than Rs. 20,000 (Rupees Twenty Thousand) for the
. District Hospitals and Rs. 15,000 (Rupees Fifteen Thousand) for the
Peripheral units, Rural Hospitals and Maternity Homes. The present
contractors should also submit their worth Certificates accordingly.
Tender forms will not be issued to those who fail to submit their
worth certificate to provide their financial status. Their tender
deposits may also be forfeited. Failure to Furnish the particulars of
worth or proof of Financial Status may Result in the Forfeiture of the
Tender Deposit.

 

4.
(i) Quantities.-Approximate quantities of supplies required
are given in Column 6 of the Schedule hereto.
(ii) Tenderers should submit rates for all types of diets (Fresh
Fish, Beef, Dry Fish, Vegetables and Eggs). As an Economic
measure the Department may decide to substitute one of the diets in
place of another, (e.g.-in lieu of an egg diet, a beef diet or fresh diet
or dry fish may be ordered if this is cheaper). However, a vegetable
diet cannot be ordered in lieu of an egg, beef and fresh fish or dry fish
diet. Contractors should note that egg, beef or fish are ordered for
lunch and vegetable for dinner and tenderers should be prepared to
supply accordingly.
(iii) The head of the institutions will be vested with the authority
to decide the number of meals that should be provided at the tendered
rates per diet. In the event of any change the tenderer will be notified
of such change well in advance to enable him to supply the required
quantity of raw materials. It should be noted that in such an eventuality.
payment will only be made for the meal or meals actually supplied.
In order to correctly compute the payment it would be essential for
the tenderer to state the rate for each meal in addition to the overall
rate for all the meals per day, in schedule ‘B’ issued together with the
tender form. The sum total of the rates for each meal should in no
way, exceed the tendered rate for all the meals as a whole for one day.

 

5. How tenders should be forwarded?-All tenders should be
forwarded in duplicate and be enclosed in a cover addressed to the
Deputy Director of Provincial Health Services, Galle. The cover
enclosing the sealed tender should bear at the left bottom corner the
name and the address of the tenderer.

 

The cover enclosing the sealed tender may be sent by registered
post or deposited in the tender box at this Office or handed over to me
or the Accountant who will issue a receipt in acknowledgement of the
tender.

 

6. Closing of Tenders.-The lenders will close at this Office at
10.00 a.m. on the date mentioned in Column 5 of Schedule hereto
and will be opened immediately thereafter.

 

7. Opening of Tenders.-Tenderers may be present when the
tenders are opened. The names of tenderers as well as the rates will
be read out to the tenderers present. Any tenderer who wishes to
scrutinize a tender will by arrangement with me or my authorised
representative, be permitted to scrutinize the duplicate of any tender
that has been submitted.

 

8. Decision offenders.-The total cost of the service arrived at on
the rates quoted for items in Schedule “B”-Diets in the tender form
will be the basis for a decision on the tenders received.
Tenderers should however, quote for items in Schedule “C”
Extras in the tender forms what the price to be paid shall be the rate
quoted in the tender or the ruling market rate which ever is lower and
‘if the price be controlled, then the controlled rate.

 

9. Tenderers should keep their officers’ open for a period of three
months from the date of closing of tenders. Tenders once submitted
cannot be withdrawn.

 

10. Validity of Tendered Rates.-The rates quoted by the tenderers
should be valid for the period of contract mentioned at paragraph 1
above, and under no circumstances will the rates be permitted to be
varied during such period, unless specially provided for in the
agreement.

 

11. Security Deposit.-The selected tenderer will be required to
sign the agreement after furnishing the required security deposit
mentioned in Column 4 of the Schedule hereto.

 

12. Storage Accommodation.-Successful tenderer will not be
provided with facilities in the nature of store room accommodation
in the institution or within its premises.

 

13. (i) The rates quoted by tenderers should be for the supply of
two cooked rice meals to a patient at 100gms.of rice per meal diet.

 

14. Tender Conditions.-Prospective tenderers should study the
tender conditions on the reverse of the tender form and comply with
the requirements there in. Tenderers should also note the conditions
of the agreement in the tender forms.
